What should I bring with me to my child’s first appointment?
When you call to schedule an appointment, please have your prescription and insurance card ready. This allows us to verify your insurance benefits and provide information about your coverage, including copayments and coinsurance payments.
If you want to save time on your first visit, please fill out our “patient forms” online or print them out before coming to your appointment.
If you don’t have time to print them ahead of time, arrive at least 25 minutes early to ensure you have enough time to complete them before your appointment.
